Greenfield Residential Home
Greenfield Home provides care and accommodation for 34 residents, aged over 65 years, who do not need hospital or nursing care but do need a safe and comfortable environment, which enhances their quality of life. A decision is made whether the client’s needs would be best met in residential care following an assessment carried out by the Care Team. A financial assessment is completed by the Care Manager and a member of the finance department to calculate the weekly charge for the client. Arrangements are made through the Care Manager for the Officer-In-Charge and Key Worker to visit the client at home or in hospital, or if possible have the client visit the home. Relevant information, the assessment documentation and care plan is shared with staff in preparation for the admission.
For emergency admissions the home requires a medical assessment and basic information. Within 2 days the Home must receive a full assessment of the client’s needs.
