Primary and Community Care

Primary and Community Care Services are responsible for the delivery of fully integrated multi-disciplinary services to patients and clients across the Trust’s geographical area. These services are based around GP practices and are delivered by a number of Integrated Service Delivery (ISD) Teams operating from health centres and other facilities throughout the Trust.

The teams are made up of:

• Nursing staff and Nursing Assistants
• Social Workers and Social Work Assistants
• Allied Health Professionals such as Occupational Therapists
• Admin and Clerical Support staff
• Treatment Room Services

Services provided:

• Community Nursing Service to 16+ years
• Social Work Service to 65+ years
• Occupational Therapy Service to 16+ years
