Our Service
We provide case management service for adults diagnosed with a long term condition including asthma/chronic obstructive pulmonary disease (COPD), diabetes and stroke. The service is Trust-wide and is in partnership with GP practices.
We hope that this service will reduce hospital admissions by 5%, with further reductions following year on year.
As well as clinics and attendances at out-patient clinics, the nurse specialists also carry out education sessions for individuals, groups and families and are also an advisory and information resource for other allied health professionals.
