The Western Trust Heart Failure Service is delivered by specialist nurses who are highly trained in caring for people with heart failure.
Heart Failure Specialist Nurses are Specialist Practitioners in Cardiology Nursing and Independent Non-Medical Prescribers.
The service is for patients who have been referred by their Consultant Cardiologist.
This service is for people who have heart failure caused by either a weakened heart pump (known as reduced left ventricular function, HFrEF), or structural changes causing a stiff heart (known as heart failure with preserved ventricular function, HFpEF). These are confirmed by tests such as an echocardiogram or cardiac MRI.
Living with heart failure can feel overwhelming, but understanding your condition is the first step in feeling more in control.
Our heart failure service aims to:
- Help you feel better and improve your quality of life
- Reduce the need for hospital admissions
- Support you and manage your condition confidently
- Provide compassionate care, including end-of-life support when needed
- At heart failure clinics, the heart failure nurse will support you by:
- Assessing your condition and symptoms
- Starting or adjusting your heart failure medications
- Monitoring your compliance and how well your treatment is working for you
- Checking for any or potential side effects
- Educating you on how to manage your condition at home
The heart failure nurses will liaise with Cardiologists, General Practitioners and other health care staff as appropriate to ensure your care is delivered to a high standard.
You will be discharged from the service when your condition has improved and you are on appropriate treatment.
