Our Service
If you or any member of your family become ill or need any kind of nursing care at home, there is a service especially designed to help you.
This service is led by a District Nursing Sister trained in giving nursing care in the community. She/he works closely with your GP and other nursing team members in planning and delivering nursing care to you and your family.
Services Offered
How Can the District Nurse Help You?
Once a referral has been made and meets the district nursing criteria, the district nurse will visit you and your family at home to discuss and plan with you, the help and care that is needed in your particular situation. This help may include one or more of the following:
- Visits by members of the district nursing team to give nursing care to someone who is acutely or chronically ill
- Visits by members of the district nursing team to give treatment required, for example, an injection or wound dressing
- Visits by members of the district nursing team to give skilled nursing care and support to patients with a terminal illness and their families
- Advice or information about health matters
- Making a referral to other professionals or agencies who may be able to help
- Giving advice, education and support to help you care for someone in your family
- Supporting someone who is recovering from an illness or disability
Who Can Make A Referral?
Referrals to district nursing services most often come from your GP or the hospital. Requests for visits by a district nurse may also be made by patients, carers and other professionals.
