We provide a range of services for disabled children and young people with a learning disability, physical disability and/or with physical health needs or sensory needs. These services aim to help disabled children and young people live lives that are as full as possible.
We use ‘Family Support’ across all our programmes for disabled children and young people, but only where it is the right and safe thing to do. We have set up a ‘Family Support panel’ involving all agencies working with disabled children and young people. Its role is to promote the best outcome for children and families based on a clear assessment of need. It provides a range of family support services that can be accessed by social workers in the Children’s Disability Programme.

- Western Education Support Team (WEST): a multi-disciplinary team including community children’s nurses, health visitors, speech and language therapists, occupational therapists, and family support workers

We work closely with a wide range of statutory and voluntary agencies to support our work with children and young people.
Short Breaks for Children with Disabilities
The Short Break Service will provide planned short breaks to families caring for children with disabilities who are known to the Children Disability Teams within the Western Trust.
