Our Service
Day Care Services are designed to meet the prescribed needs of older people through individualised care plans – structured comprehensive programmes of care that provide a variety of health, social and related support services in a protective setting for a period of time during the day.
Day care services are available to adults over 65 years and younger adults with physical disability who have been assessed by their keyworker as in need of a prescribed service at level 1 – regional access criteria.
The Trust operates six main Day Centres which operate Monday – Friday and six outreach centres that operate 1 – 3 days per week. The Trust also commissions Day Care Services from the community and voluntary sector who provide day care services in locations across the Trust’s catchment area.
