What is ‘Enhanced Intermediate Care’?
‘Enhanced Intermediate Care’, or EIC, is the name of a scheme for patients who are well enough to go home from hospital but who have very complex needs. These patients will be making life changing decisions about their future care environment. It aims to provide that support for about two weeks after leaving hospital. The referal guidelines are as follows:
- Medically fit
- Multi-disciplinary Team assessed (Nursing, Social Work, Occupational Therapy etc)
- Undergoing comprehensive assessment
- No further rehab goals etablished in acute stages
- Aged 18 plus
- Referrals from – Altnagelvin Hospital and Waterside Hospital
- Pilot commence City / Waterside localities (Initially)
- Making life changing decisions.
What are the benefits of EIC?
The EIC scheme means that you don’t have to stay in hospital any longer than you need to. It also means that you and your family will have more time to make important decisions about any ongoing care you might need in the comfort of your own home.
Am I suitable for EIC?
A member of the EIC team will meet with you and your family to decide if EIC is right for you. They will keep you informed of all progress.
What happens if I am suitable?
The EIC Team will work closely with the different professionals in hospital to find out what support you will need at home. You and your family will be involved throughout. They will order any equipment and organise any help you need with washing, dressing, cooking or lighting fires, etc.
If you need a place in a nursing or residential home, they will help to arrange that for you. And they will make sure social services and the district nursing team provide whatever services you need when you leave hospital.
Their overall aim will be to work closely with you and your family to help you make the right choices about any care and location of your care you will need in the future.
Will I have to pay for this service?
The EIC service is free if you go to your own home or a relative’s home. If you have to go to a nursing or residential home, you may have to pay part, or all, of the costs. You will be informed in advance how much it will be.
What happens at the end of the two weeks?
After two weeks, the EIC team will meet with you and your family to see how you are doing and talk about any future care you might need. A social worker and district nurse may attend this meeting.
