Domiciliary Care
Domiciliary care is provided on the basis of assessed need to enable the individual to live at home in the community and to be supported in maintaining critical personal care and daily living tasks. Currently this is offered on the basis of critical need and allocation of the service is managed through a Community Access Panel. Domiciliary Care is provided through Trust Home Care services and a number of independent providers.
Psychological Therapies Services
The Learning Disability Psychological Therapies Service incorporates Clinical Psychology, Behaviour Support and Intensive Support. We provide an holistic therapeutic service which is person-centred incorporating psychological assessments and interventions specific to client need. The service aims to enhance the psychological well-being of our individual service users across all sectors of the programme. We also aim to increase psychological mindedness among families, carers and professionals as we support all those who work with us to optimise quality of life for individuals with a learning disability within our Trust.

Psychiatry
Psychiatry services are available for people with a Learning Disability. Consultant Psychiatrists operate inpatient and outpatient service support through Lakeview Hospital and community clinics.

Community Teams
Community Social Work Teams
The role and function of the Community Teams within Adult Learning Disability is to discharge statutory functions and the management of cases appropriately through care management, key working and PQC processes with due attention to the meeting of need and the Safeguarding of Vulnerable adults.
The Community Teams are situated across four localities within the Trust and provide a link/co-ordinating role for service users and their carers in the amount of need, delivery and monitoring of care packages. The Community Team also consists of Forensic Social Workers, Transition Officers, Family Support and Floating Support Workers.
Community ALD Nursing Teams
The Community Learning Disability Nursing Team is based in various locations across the Western Trust area. The Service aims to utilise all learning disability nurses to target health inequalities and health facilitation will be an integral aspect of the work of community learning disability nurses going forward to inspire the health outcomes of adults with a learning disability in partnership with GP practices.
The Community Learning Disability Nurses manage a referral caseload, carry out health assessments, offer care and treatment plans, contribute to diagnosis and health monitoring, facilitate health and well-being plans, offer specialist knowledge and empower, support and advocate.
