1. Referral by GP to determine if Learning Disabilities Present, then Assessment by Consultant Psychiatrist/Clinical Psychologist
1a. Learning Disability Diagnosis – Yes , then Refer to Community Social Work Team, then Assessment of Need and Carer’s Assessment Undertaken, then Care and Support Plan developed, then Services provided based on assessment of need, then Annual Case/Care Review or earlier if required.
1b. Learning Disability Diagnosis – No, then Discharge and Signpost
OR
2. Referrals by Children’s Services through transitions process.
2a. Learning Disability Diagnosis – Yes, then Refer to Community Social Work Team, then Assessment of Need and Carer’s Assessment Undertaken, then Care and Support Plan developed, then Services provided based on assessment of need, then Annual Case/Care Review or earlier if required.
