The team provides specialist respiratory nursing care to patients in their own homes and at local clinics. The service aims to reduce avoidable hospital admissions, support patients to leave hospital safely and earlier where appropriate, and improve the diagnosis and management of respiratory conditions.
The Community Respiratory Team operates from Omagh Hospital and Primary Care Complex, Gransha Park and South West Acute Hospital, providing locally accessible specialist services including:
- Community Respiratory Team assessment and review
- Home Oxygen Service assessment and review
