Community Respiratory Team

You have been referred by your General Practitioner, or by a hospital, to the Community Respiratory Team for assessment, management, support and guidance with your lung condition.

What services does the team provide?

Supported discharge and hospital in-reach

This service supports respiratory patients to leave hospital as soon as it is safe to do so.

Patients are assessed in hospital and, if they meet the criteria for the service, they are discharged home and reviewed by the Community Respiratory Team until they return to their usual level of health.

This support may include:

  • a home visit
  • a telephone review
  • an appointment at a local clinic.

Support in the management of lung disease

This service is for patients who are experiencing a worsening of their chronic lung disease.

The aim is to prevent unplanned hospital admissions and, where appropriate, support patients to manage their condition at home.

Self-management plan

The team can help develop self-management plans in line with National Institute for Health and Care Excellence and Global Initiative for Chronic Obstructive Lung Disease standards.

Other services

The Community Respiratory Team also provides:

  • oxygen assessments and reviews
  • pulmonary rehabilitation
  • community Chronic Obstructive Pulmonary Disease clinics.

Your care

The nursing teams are highly skilled and will contact your doctor for advice or assistance if they have concerns about your condition.

If you have any questions or concerns, please speak to a member of the nursing team.

If you need to be readmitted to hospital at any time during your treatment, the nurse will speak to the hospital consultant or your General Practitioner to assist with your admission.

Complaints and feedback

If you have a complaint, please speak to the nurse looking after you.

Alternatively, you can contact the Trustwide Community Respiratory Team Leader, who can explain the complaints procedure to you.
