Our service is to manage the programme of work developed across the Western Trust, to implement the Department of Health strategy “Health & Wellbeing 2026” – Delivering Together
Funding has been made available to all Trusts to transform how, where and by whom health and social care is provided.
The transformation will take place across five areas:

- Providing more support in primary care
- Reforming our community and hospital services
- Elective care waiting lists
- Organise ourselves to deliver
