Following a CHOICE assessment if it agreed together that another service more suited to meet your needs, an onward referral will be discussed.
After your assessment if it’s agreed that further support from CAMHS is required, we’ll work together to create a care plan – this is just a clear plan of what help and support will be most useful for you.
This might include:
- Regular individual and/or family appointments with someone from our clinical team
- Working in partnership with you and your family to achieve your goals agreed in the care plan
