From Monday 1st June 2026, the Western Trust’s Rapid Response and District Nursing Services will integrate and operate as a single, coordinated community nursing service.
This new service will be known as the Community Nursing Home and Hub Service. By bringing these well-established teams together, the Trust will provide a more streamlined and responsive service for our community, ensuring continuity of care and better outcomes for those we support.

Assistant Director of Primary & Community Care, Paula Devine, said:
“District nurses and rapid response nurses play a vital role in improving patient outcomes by delivering immediate, specialised care directly where it is needed. Together, they help bridge the gap between acute hospitals and local communities, preventing unnecessary hospital admissions, easing pressure on emergency departments and enhancing overall patient safety.
“I am extremely proud of the dedication and hard work shown by staff at every level as we developed this model together. It has been truly inspiring to see everyone collaborating so closely and supporting each other, always keeping the best interests of our patients, families, and carers at the centre of what we do.
