Our service is to manage the programme of work developed across the Western Health and Social Care Trust, to implement the Department of Health strategy “Health & Wellbeing 2026” – Delivering Together
Funding has been made available to all Trusts to transform how, where and by whom health and social care is provided.
The transformation will take place across 5 areas:
- Building capacity in communities and in prevention
- Providing more support in primary care
- Reforming our community and hospital services
- Elective Care Waiting Lists
- Organise ourselves to deliver
